Subject’s Consent to the Processing of Personal Data

I (hereinafter – «Subject»), provide Consent to the processing of personal data (hereinafter – «Consent») under the terms set forth below.

1. The Subject gives consent to the Operator for the processing of the following personal data:
   1. Last name, first name, patronymic, gender;
   2. Date of birth;
   3. Email address;
   4. Phone number;
   5. Other information processed by the Operator:
      1. Data about technical means — IP address, type of operating system, browser type, geographical location, internet service provider; information about Site usage.
2. Processing of personal data by the Operator means an action (operation) or a set of actions (operations) with personal data, including collection, recording, systematization, accumulation, storage, clarification (updating, modification), extraction, use, transmission, anonymization, blocking, deletion, destruction of personal data.
3. The Subject gives consent to the processing of personal data for the following purposes:
   1. Identification of the Subject;
   2. Interaction with the Subject, including sending notifications, requests, and information regarding the actions of the Operator, as well as processing requests and applications from the Subject;
   3. Sending advertising materials and information about special offers to the Subject;
   4. Ensuring the proper functioning of the Operator’s website;
   5. Preparing documents confirming payment of services by the Subject and other documents related to the Subject’s actions on the Operator’s website;
   6. Facilitating interaction between the Subject and the Operator for the purpose of providing services;
   7. Compliance with regulatory and non-regulatory legal acts, as well as decisions, assignments, and requests of government authorities and individuals acting on behalf or at the request of such authorities;
   8. Analyzing the interests of the Subject and conducting surveys;
   9. Ensuring the necessary level of security.
4. The Subject has the right to submit a request to the Operator for clarification of their personal data, or to demand blocking or deletion if the data is incomplete, outdated, or inaccurate.
5. The Subject consents to the transfer of their personal data by the Operator to third parties for the provision of services requested by the Subject on the Operator’s website, or for similar fulfillment of the agreement between the Subject and the Operator, as well as for the purpose of conducting audits.
6. The Subject’s personal data will be processed until the liquidation of the Operator.
7. The Operator processes the Subject’s personal data in accordance with local regulatory acts.
8. The Operator takes necessary and sufficient organizational and technical measures to protect the Subject’s personal information from unauthorized or accidental access, destruction, alteration, blocking, copying, distribution, as well as from other unlawful actions by third parties.
9. This Consent can be revoked by the Subject or their representative by submitting a written request. The Consent may be revoked provided that the Subject notifies the Operator at least 30 days prior to the intended date of cessation of data processing.
10. In the event of revocation of this Consent by the Subject or their representative, the Operator has the right to continue processing personal data without the Subject’s consent if there are grounds specified in clauses 2-11 of Part 1 of Article 6 and Part 2 of Article 10 of Federal Law No. 152-FZ «On Personal Data» dated July 27, 2006.
11. This Consent is valid from the moment it is provided and during the period of consideration of the User’s inquiries or until the Consent is revoked.
